Longevistic Terms of Service (Membership Terms)
Effective date: October 1, 2026
These Terms are an agreement between you and Washington Specialty Physicians, Inc., doing business as Longevistic (“Longevistic,” “we,” “us”). They cover the public website longevistic.com, the member portal at portal.longevistic.com, and Longevistic memberships. You accept these Terms at the end of the intake questionnaire by ticking two required checkboxes and selecting Submit. There is no typed name. Ticking the boxes and selecting Submit is your electronic signature, and we record your account name and email, the date and time, and the version of these Terms. Payment is separate from the consent step.
The first checkbox is your agreement to these Terms, the Privacy Policy, the Washington Consumer Health Data Policy, and the HIPAA Notice of Privacy Practices, and your consent to telehealth care. The full agreement under that checkbox covers evaluation and care, telehealth, messages, health records, and costs.
The second checkbox is separate because Washington’s My Health My Data Act requires sharing consent to be separate. It reads: “I consent to Longevistic sharing my health information with Fullscript and its lab partners to order and deliver my labs and supplements.” Labs are part of this agreement, not merely a disclosure. Fullscript is how Longevistic orders labs and supplements. You can withdraw that consent in the portal Account tab or by emailing hello@longevistic.com. If you withdraw it, labs and supplements cannot be ordered through Fullscript.

1. What Longevistic is (and is not)
Longevistic is a physician-led preventive and longevity medicine practice. Depending on your plan, membership includes testing, a Longevistic Score™, a written plan, and physician visits.
- Not emergency care. Do not use the portal, email, or messages for urgent or emergency needs. Call 911 or go to the nearest emergency room.
- Not a replacement for primary care unless we tell you in writing that we will serve as your primary care provider.
- Not insurance. Membership fees are not health insurance, do not satisfy any insurance mandate, and are not billed to insurance.
- Results are not guaranteed. Health outcomes depend on many factors. The Longevistic Score™ and plans are clinical tools, not promises.
2. Eligibility
- You must be at least 18.
- Physician services are provided by clinicians licensed in Washington. Telehealth visits require you to be physically located in Washington during the visit.
- Give accurate, complete information and keep it up to date.
- We may decline or end a membership where care is not clinically appropriate, as permitted by law and professional rules. We will help with referrals where appropriate.
3. Your account
Keep your password secure; you are responsible for activity on your account. Tell us promptly at hello@longevistic.com if you suspect unauthorized use. One member per account.
4. Membership plans and prices

Billed separately (not included in any plan): laboratory tests and imaging, specialist referrals, prescriptions and pharmacy costs, and supplements. Labs and imaging may be billed by us or by the lab or imaging center, and we will tell you the price or billing party before ordering.
Prices may change. We will notify you before a renewal at a new price.
5. Payment
Payments are processed by Stripe. We do not store your full card number. By paying, you authorize us, through Stripe, to charge the plan price and any applicable tax. For annual plans, this includes each automatic renewal payment described in section 6 until you turn off auto-renew or cancel.
6. Renewal, cancellation, and refunds
Annual plans (Autopilot, Copilot, Charter)
- Automatic renewal: annual plans renew automatically for another 12-month term at the then-current price, charged to your payment method on file on the renewal date.
- Turn off auto-renew or cancel anytime; it takes effect at the end of your current paid term. To avoid the next renewal charge, make the change before the renewal date. Use the portal (Membership tab) or email hello@longevistic.com. Your access and included services continue until the end of the paid term, and you will not be charged again after that term.
- We will email you before each renewal with the renewal date and amount.
- Cancellation does not shorten the current paid term. Fees for the current term are not refunded except where required by law.
Runway (one-time)
- Runway is a single paid cycle. Nothing renews.
- Refundable until your lab kit is ordered. You can cancel Runway for a full refund any time before your lab test kit is ordered. Once the kit has been ordered, Runway cannot be refunded. To cancel, email hello@longevistic.com.
Plan changes
Before payment, you can switch plans in the portal. After payment, contact us through the portal or at hello@longevistic.com to request a change. We will confirm any approved change and applicable price difference by email.
Our right to end a membership: we may end a membership for non-payment, abusive conduct toward staff, fraud, or when continuing care is not clinically appropriate, with notice and help transitioning care where appropriate. We will provide any refund required by law.
7. Telehealth and communications
- Visits are always virtual. You must be physically in Washington during the visit. If a condition needs an in-person exam, Longevistic refers you out. Technology can fail.
- Portal and email messages are not monitored around the clock. We respond as soon as reasonably possible. For anything urgent, call (206) 486-0662; for emergencies, call 911.
- Our intake consent covers telehealth and electronic communication in more detail.
8. Supplements and Fullscript
- Fullscript is how Longevistic orders labs and supplements. Fullscript and its lab partners fulfill those orders under their own terms and privacy policies.
- Labs are part of the Fullscript sharing agreement, not merely a disclosure. The agreement is the separate intake checkbox: “I consent to Longevistic sharing my health information with Fullscript and its lab partners to order and deliver my labs and supplements.” You can give that consent at the end of intake or later in the portal Account tab, and you can withdraw it in the Account tab or by emailing hello@longevistic.com. If you withdraw it, labs and supplements cannot be ordered through Fullscript.
- You are never required to buy supplements through Fullscript or from us.
9. Your health information
Our Privacy Policy, Consumer Health Data Privacy Policy, and Notice of Privacy Practices explain how we handle your information. Health data for the member portal and care systems is stored with AWS under a BAA. The public website is hosted separately and does not store PHI.
10. Acceptable use
Do not misuse the Site. That includes attempting unauthorized access, interfering with security, uploading malware, scraping, impersonating others, or using the Site unlawfully. We may suspend accounts that do.
11. Intellectual property
The Site, the Longevistic name and marks, the Longevistic Score™, and our content belong to us or our licensors. Your written plan is for your personal use. You own the information you give us; you let us use it to provide services as described in our privacy documents.
12. Disclaimers
Apart from our professional obligations as health care providers, the Site and portal are provided “as is.” We do not guarantee uninterrupted or error-free operation. General content on the Site, including articles and newsletters, is educational and is not medical advice for your situation.
13. Limitation of liability
Nothing in these Terms limits liability that cannot be limited under applicable law, including liability for professional negligence. To the fullest extent permitted by law, Longevistic will not be liable for indirect, incidental, special, consequential, or punitive damages arising from use of the Site or portal.
14. Disputes and governing law
These Terms are governed by Washington law. Disputes will be handled in a court of competent jurisdiction.
15. Changes to these Terms
We may update these Terms. We will post the new version with its effective date. For material changes, we will notify members in advance. Changes apply from your next term unless the law allows sooner.
16. General
If a provision is unenforceable, the rest remains in effect. These Terms, the plan description, and our privacy documents are the whole agreement about the Site and membership. We may assign these Terms in connection with a sale or reorganization of the practice.
